You will need to complete the following forms:

  • Contractor Registration Form (doc 949kb)
       
  • Contractor Questionnaire (doc 823kb), unless your organisation is exempt because:
    • Annual turnover is greater than AUD$800,000;  or
    • University, TAFE or other education institution; or
    • Commonwealth or State Government Department; or
    • Company is registered outside Australia.

Each organisation must notify AWI immediately if any element of its registration, including the Contractor Questionnaire changes.

Guidelines for completing the Contractor Registration Form

Australian registered organisations - please complete all items in the Registration Form and clearly identify where you are providing supporting documentation. 

1 Organisation Name

 Enter the legal name of your Organisation, including any other name it may trade under.

2 Organisation Type

Tick the box identifying the legal status of your organisation.

3 Organisation Details

Enter all contact information as required, including an ABN or ACN.  Where your organisation does not have an ABN or ACN write “Not applicable”.

4 Contact Person Details

Enter the name and contact details of the person in your organisation who AWI can contact in regard to this registration and in regard to future contracting opportunities.

5 Organisation Areas of Expertise

Tick the box as appropriate that reflects your organisation area of expertise.  The definitions of each area are as follows:

  • Sheep and Wool Biology
    Sheep growth and physiology, including reproductive pysiology, Wool follicle development, structure, and function; Fibre quality; Rumen structure and function.

You can tick more than one box, however you must be able to demonstrate that you have expertise in each of the areas you have identified (see 6).

6 Recent Products and Services provided

For each area of expertise that you identified in 5, you must provide evidence of work that you have recently completed to support your claim of expertise.  Please identify the product/service you delivered, when it was delivered and who you delivered it to.

7 Key Personnel

Enter the name, job title, area of expertise and contact details for key personnel in your organisation.  You may include as many key personnel as you wish, however your key personnel will most likely be senior employees with significant industry experience.

8 Organisation Rates

For each key personnel identified in 7, list the hourly and daily rates in Australian dollars (AUD$).  Clearly identify any differential in rates between casual (one-off / as required basis) or full-time (project full time) basis.

9 Organisation Availability

Enter the number of full-time equivalent employees you have.

10 Organisation Referrees

Identify at least 1 Referee that AWI can contact.  You need to include their contact details and a summary of the work you performed for them and when.

11 AWI Contracting Statement

AWI uses a standard suite of Contracts with External Parties.  These are available here.

Identify your acceptance or otherwise to use these standard contracts.  Where your organisation disagrees, please provide commentary on why.

12 Contractor Questionnaire

All organisations seeking to register with AWI must complete and submit to AWI a Contractor Questionnaire unless:

  • Annual turnover is greater than $800,000;  OR
  • Organisation is a State or Commonwealth Government Department: OR
  • An educational institute such as a University or a TAFE; OR
  • Organisation does not have an ABN or ACN.

13 Date Form Completed

Enter the date the form was completed (DD/MM/YYYY).

Disclaimer

At its absolute discretion, AWI reserves the right to:
a) Register an organisation with or without conditions; or
b) Reject an organisation’s application for registration; or
c) Suspend an organisation’s registration; or
d) Remove an organisation from its Contractor and Consultant Register. 
 
AWI will not be held liable for any costs or damages incurred by an organisation in the exercise of such discretion.

Privacy

The information provided to AWI will only be used for the purpose of enabling AWI to easily identify organisations that have the necessary skills to undertake project and consultancy work.  The Register will be accessed by AWI employees and AWI Contractors who are required to identify suitable organisations to obtain quotes from, as part of their contracted duties.
